At Community Care Connect, our mission is to empower seniors and their families in California to make informed decisions about their housing and care options. We serve as a bridge between seniors and various types of facilities, providing independent information and analysis to help them find the best solutions for their needs. Through our community-funded agency, we collect and share information on different types of facilities, insurance programs, and levels of care, ensuring that seniors can access affordable, high-quality housing and support services.

With all the advancements in technology and the way we live our day to day lives, it feels as though the needs of many members of our communities have slipped through the cracks. Due to various circumstances such as age, language barriers, and socioeconomic status, there are a number of people who feel disconnected and unable to explore resources and determine their care needs. At Community Care Connect, we firmly believe that regardless of how old you are, what language you speak, or where you live, you have a right to resources, and more importantly, information. Despite those beliefs, we have found there to be a gap in the community leaving many confused and lost when it comes to their or their loved ones care whether it be in regards to aging or assisted living, addiction, or mental health. CCC wants to close that gap by offering the community and its members a safe haven for questions and concerns, and lending a helping hand by providing the HELPFUL information for understanding, determining, and addressing every individual’s specific needs.
